What it is for
A job safety analysis (JSA, sometimes JSEA with the environment added) breaks a task into its steps and asks the same three questions of each: what could go wrong here, what will we do about it, and who owns that. It is the quickest way to get a crew thinking about a job before they pick up the tools.
This template keeps the layout simple enough to fill in at the ute. Job details and permits at the top, a PPE checklist, then a numbered step table with hazards, controls and a responsible person. The sign-on sheet at the bottom records who was part of the conversation.
What is in the template
- Job details: job, location, date, prepared by, supervisor and permits required.
- A PPE checklist for the task.
- A ten-step table: step number, job step, hazards, controls and responsible person.
- A crew sign-on sheet with name, signature and date.
How to use it well
- Do it with the crew, not for them. The people doing the job know the hazards the office does not.
- Keep steps to what a person actually does: set up, isolate, lift, cut. One verb per step.
- If a step needs a permit or a licence, write it in the controls so the check happens before the step, not after.

Before you use it
This template is a general starting point only. It is not advice, it was not prepared for your workplace, and it does not take account of your sites, your work or the requirements that apply to you. You are responsible for checking it, adapting it and deciding whether it is suitable before you use it. AREESA accepts no responsibility or liability for any loss, injury or damage arising from its use.
When the paper version stops scaling
In AREESA a JSA is a form the crew fills in on their phones, with photos, signatures and a cascading question that opens follow-ups when an answer is no. Every completed JSA is time-stamped, signed and searchable by person, site and date.
Questions
JSA or JSEA, which should I use?
They are the same document. JSEA adds environmental hazards to the analysis. Use whichever your clients and your industry expect; this template works for both.
When do I need a JSA instead of a SWMS?
A SWMS is for high-risk construction work and goes to the principal contractor. A JSA is for any task where a step-by-step think-through helps, on any site. Many crews use a JSA daily and a SWMS for the high-risk jobs.
How many steps should a JSA have?
As many as the job has, usually between five and twelve. If you are past fifteen, the job probably needs two JSAs.
Can I keep the JSA on a phone?
The Word and PDF versions are for printing or filling in on a laptop. For a JSA that runs on phones with signatures and photos, that is what the forms module in AREESA does.
